Reports lived in chats and on paper
Across multiple industrial sites, hazard reports arrived as WhatsApp messages, verbal mentions and paper forms.
Reports went missing, follow-ups had no owner, and management had no picture of what was open, overdue or repeating.
One report, followed to closure
A worker spots a hazard, takes a photo and files it from the phone in under two minutes. The HSE officer is notified on WhatsApp, rates it on the risk matrix and assigns a corrective action with an owner and a due date. Management sees it close, verified, with every step in the audit trail.

A public API
Token-authenticated, so other internal systems can submit and read reports.
Engineering notes
Next.js 16 with MySQL, installable as a PWA with web push. Role-based access separates field reporters, HSE officers and management.
Every state change is logged, and the audit trail is append-only, so a closed report cannot be quietly rewritten. Email flows run through Brevo, WhatsApp through the Meta Business API.
